I would like to inform you that as of 15.09.2009 a new price list with new services will be in force at the clinic. The price list contains approximate prices. The treatment plan and cost estimate is prepared by the attending physician after assessing the condition of your oral cavity and learning your expectations.
We accept cash only in payments for services. A receipt is the confirmation of payment. We draw up invoices within 7 days from providing the service upon presenting the receipt. There is a possibility of obtaining a transfer invoice at the request of the patient.
Considering the best interest of patients waiting for visits, please cancel your visits at least one day prior to the scheduled visit.
In case you confirm the visit and do not appear, you will be charged with the cost of blocking the office at a rate of PLN 50 for each 30 minutes.
